Declaration

I hereby declare as follows

1. That the information contained here is true and correct.
2. That my application to become registered with Coastline Cleaning, no way contravenes any pre-existing contract or agreement with any personnel agency or employer.
3. That I am being engaged as follows: -
a. As a Casual Employee I will be paid as such. I have no expectation of continuing employment, nor offers of permanent full time or part time employment.
b. As a casual employee I will notify the employer of my availability on a weekly basis in advance.
c. I will be advised of the approximate number of hours I am required to work prior to commencement of each shift.
d. As a casual employee I will receive a rate of pay that includes loading to compensate for the casual nature of the work, sick leave and annual leave
4. That I hereby undertake not to approach any client nor accept any approach from any client, subsidiary to a client, or customer referred to me by Coastline Cleaning, with respect to obtaining or providing my services for a period of at least 6 months after the last occasion on which I performed services for or was referred to that client, unless the approach was made through Coastline Cleaning. I further agree that should I perform services for such a client in breach of this provision, I will pay to Coastline Cleaning a referral fee of Five Thousand Five Hundred Dollars ($5,500) including GST or such referral fee as is subsequently set by Coastline Cleaning after the date of this agreement, immediately such services are provided or an agreement is made for provision of such services.
5. I hereby authorise the department of Immigration and Multicultural Affairs (DIMA) to release details of my work rights status (that is, my entitlement to work legally in Australia) to Coastline Cleaning and I understand these details are held by DIMA on departmental files and computer systems. I further understand that Coastline Cleaning will use this information for the purposes of establishing my legal entitlement to work in Australia, and for no other purpose. I also understand that I allow release of my work rights status information for a period of three months from the date of this application.
6. That my personal details and visa details are correct.
7. I, the undersigned, have read this application in full and by my signature below do attest to my understanding of such, and my agreement to all of the provisions and conditions contained herein.
